You must be at least 13 years of age to complete the Finalist portion of this form. If you are under 13 years of age, you must have a parent fill out the entire form.

The Parent/Guardian section of the form must be completed by a Parent or Guardian regardless of your age. Please continue with registration if the following statements are true: 
1) I am at least 13 years of age, or my parent is completing this form.

Soloist Song Request

The song request submission deadline is July 1 and song backup music submission deadline is July 15. Your song request will be honored on a first come first served basis and you will be contacted with a verification of your song choice. It can be changed any time up to July 1.

~mp3 or m4a files emailed are the preferred method of delivery for your accompaniment music. Please email your mp3/m4a karaoke track to lamorindaidol@gmail.com.

Parent/Guardian Information

This page is to be filled out by the primary volunteer/guardian of finalist. This will be the primary contact for all email correspondence from the Lamorinda Idol staff.

Parent/Guardian 2 will also receive copies of emails sent to the finalist.

I take full responsibility for any injury incurred. In case of emergency, call the number listed in this form. If I cannot be reached, I authorize Lamorinda Arts Council member to call our family physician or emergency personnel. I understand that Lamorinda Arts Council WILL NOT provide insurance.

Parent Volunteer Responsibility

Parents are an integral part of Lamorinda Idol, and we could not offer this program without your help. We ask that each parent volunteer a minimum of 5 hours to support the program. There is an "opt out" payment option as well.

We will try to assign you to one of your areas of interest. our goal is to share the work as equitably as possible and maximize the benefits of our collective talents and experience. Please enter below any special talents or previous experience you have.

Pool Party

Pool Party Planning and Set-Up: Organize and set up Pool Party on June 17.

Pool Party BBQ: BBQ at Pool Party on June 17.

Pool Party Clean-Up: Clean up at Pool Party on June 17.

June 30 Lafayette Plaza Performance 

June 30 Set-up: Assist in set-up of equipment before June 30 performance. 

June 30 Performer Management: Assist management of performers at June 30 performance. 

June 30  Pack-up: Assist in pack-up of equipment after June 30 performance. 

June 30 Photography: Take photographs at June 30 performance.

July 18 Orinda Park Performance

July 18 Set-up: Assist in set-up of equipment before July 18 performance. 

July 18 Performer Management: Assist management of performers at July 18 performance. 

July 18  Pack-up: Assist in pack-up of equipment after July 18 performance. 

August 13 Theatre Square Performance

August 13 Set-up: Assist in set-up of equipment before August 13 performance. 

August 13 Performer Management: Assist management of performers at August 13 performance. 

August 13  Pack-up: Assist in pack-up of equipment after August 13 performance. 

Poster Distribution

Assist Publicity Team in distribution of Finals posters during July and August.

Gift Certificate Solicitation

Obtain gift certificates in July and August to be given to judges, etc. at Finals.

Rehearsal and Finals

Rehearsal and Finals Chaperone: Serve as chaperone for one category of singers at the rehearsal on August 25 and the Finals August 27. Volunteers must be available both dates.

Rehearsal Set-Up: Assist in setting up equipment for rehearsal for Finals on August 27.

Rehearsal Pack-Up: Assist in packing up equipment following rehearsal for Finals on August 27.

Rehearsal Refreshments: Provide snacks and refreshments for rehearsal staff on August 25.

Finals OIS Equipment: Pick up tables from OIS and deliver them to Orinda Theatre on August 25.  Pick tables up after Finals on August 27 and deliver them to OIS on August 28.  Must have large van or truck.  

Finals Set-Up: Assist in setting up equipment for Finals on August 27.

Finals Photography: Take photographs at the Finals.

Finals Pack-Up: Assist in packing up equipment following Finals on August 27.

After Party

After Party Hosting: Provide home for After Party on August 27, following Finals.

After Party Planning and Set-up: Organize and clean up following After Party on August 27, following Finals.

After Party Clean-up: Clean up following After Party on August 27, following Finals.

Opt Out: Opt out of volunteer role by paying $150 fee.

Other Please specify if you are interested in a leadership role on the Lamorinda Idol Committee.

The fee for Finalists is $200 per family. Additional contributions are welcome.   
 
Note that this year there will be no Good Luck Ads in the playbill in order to simplify the playbill production.
 
Parents of Finalists who chose not to provide volunteer hours will pay a Volunteer Opt Out Fee of $150.

Finalist Acknowledgement and Agreement to Terms of Acceptance
